AdmissionsWe
enroll children between the ages of 12 months and 3 years (at start of enrollment).
The children can then stay until they start Kindergarten, renewing the Enrollment
Agreement each year. The
following steps are required for admission: 1.
Call
to make an appointment to visit. 2.
If
you are interested and there is a spot available for the schedule you need, fill
out the Family Information,
Medical Consent
and Enrollment
Agreement forms and have your pediatrician fill out the Confidential
Medical Record required by the NYC Department of Health. 3.
Submission
of the above forms plus a $200 non-refundable registration fee will hold a spot
at Carolyn’s Play House. 4.
A short,
hourly Trial Period then begins, where your child, you, and our staff assess the
adaptibility of your child to CPH without the commitment of a binding contract.
5.
At
the end of the Trial Period, the parents sign the Regular Period Enrollment Agreement
with a commitment for the rest of the school year (September through August). 6.
In
May of each new school year, the parents can sign up for the coming school year.
